Preparing for your first online appointment

You must be physically located in the State of Pennsylvania during your session

1. Create a Patient Portal Account

Upon confirmation of your appointment, we will send you an email link to sign up for an account. You will use this account to request appointments, join video sessions , sign consent forms , review privacy statements and send secure messages. We use Charm Health for our patient portal.

2. Download and Install Zoom Meeting

Click here to join a test meeting. This will automatically download the secure app to your computer, iPhone, or Android device. You don’t need to create an account with zoom, just install it. Test your internet connection for >10 mb/sec download speed.

3. Review and sign Patient Questionnaire

Make sure you review and sign all consent forms & privacy policy statements. Also complete the intake form in advance.You can find them in the questionnaire section of patient portal or follow the instruction the in e-mail. You can complete them any time after receiving them but prior to your appointment.

4. Find a Private Space for your Session

We recommend that you find a private space for your session where others can’t hear you. Also write down any questions you would like to discuss with the psychiatrist.

5. Joining the Zoom Meeting Session

There are two ways to join the Zoom meeting. 1) Log into patient portal, go to appointments, then check-in. or 2) click on the link in the e-mail sent to you. We recommend you log-in to your patient portal at-least 30 minutes before your session and complete your intake forms, if not done already.

6. Get your Prescription directly to your Pharmacy

After your psychiatrist completes your evaluation, he or she will send prescriptions electronically to your pharmacy. Please make sure to provide the most updated pharmacy information.
